Employers with 20 or more employees are required to offer Federal COBRA Continuation Coverage. States, such as Pennsylvania may also require employers with fewer than 20 employees to offer a State regulated form of "Mini-COBRA". - Qualifying Event - A notice must be provided to any covered employee or dependent who experiences a loss of coverage due to a Qualifying Event such as a termination of employment, reduction of hours, divorce or loss of dependent status.
- Initial Notice- Under Federal COBRA, an Initial COBRA Notice must be furnished to any employee and/or spouse (when enrolled) upon commencement of coverage under the group health plan.
